About 2.12 million Medicare Advantage members will be auto-enrolled into a different plan on January 1, 2027. They sit in 312 MA plans across 44 states and 2,335 counties, and most will land in their new plan without making a choice.

A new plan often means a new plan name, a new benefit design, or a new contract, and members who don't open their Annual Notice of Change may not realize anything moved.

By Plan Type

The moves skew toward HMO and D-SNP products.

HMO/HMO-POS plans hold 81% of affected members (1.71 million), Local PPOs 19% (403,250), and a single PFFS plan the rest.

By SNP type, dual-eligible plans account for 1.17 million members (55%), non-SNP plans 926,287 (44%), C-SNPs 21,777 and I-SNPs 1,132.

Most of the movement is consolidation. Plans CMS classifies as Consolidated Renewal Plans hold 1.08 million affected members (51%), straight Renewal Plans 566,955 (27%), and renewals with a service area reduction (SAR) 217,144 (10%).

In 76% of affected county records, only one crosswalk plan is offered, so the member's landing spot is unambiguous; the other 24% have multiple crosswalk plans and assignment depends on eligibility.

Who's moving members

UnitedHealth Group alone accounts for 41% of auto-enrolled members, nearly double Humana. Forty-two parent organizations have at least one plan crosswalking members to a new ID.

Elevance runs almost as many affected plans as Humana (49 vs. 45) but with smaller plans, about 2,200 members each on average. At the other end, Point32Health's single Tufts plan moves nearly 50,000 members.

Where the moves land

Texas has the most auto-enrolled members at 241,091, followed by Tennessee and North Carolina. Twelve states each top 68,000 affected members.

Plan counts and member counts tell different stories. Tennessee's 179,850 members sit in just five plans, mostly large Humana and UnitedHealthcare products, while Florida spreads 121,857 members across 52 plans, the most of any state.

The biggest moves

Fifty-seven plans move more than 10,000 members each, and the ten largest account for a quarter of all affected members. Two UnitedHealthcare D-SNPs lead: Texas and North Carolina together move about 175,000 dual-eligible members.

Also in the file: eligibility splits

A separate group of 122 MA plans, about 1.08 million members, falls into an "eligibility split," where the 2026 plan continues in the county and some members stay while others move to a crosswalk plan based on eligibility. Every one of these is a dual-eligible SNP, concentrated in Florida (376,039 members), Indiana and Massachusetts, and led by UnitedHealth Group, Humana and Elevance.

Methodology: MMI+ analysis of the CY2027 crosswalk at the county level, Medicare Advantage only (MA-PD and MA-Only); standalone Part D plans are excluded. Enrollment is October 2026.
