Humana’s NTM Commission Change

Humana recently implemented a notable change to its Medicare Advantage commission structure.

Historically, New-to-Medicare (“NTM”) enrollments received the full initial year commission, regardless of the enrollment month.

Humana’s new policy prorates NTM commissions based on the enrollment month, aligning them more closely with replacement policy commission structures.

While the change may appear small operationally, it has meaningful implications for both Humana’s commission expense and distributor economics.

In this analysis we estimate:

  • The potential reduction in Humana’s commission expense

  • The impact to distributor policy lifetime value (LTV)

  • How the change alters cash flow timing for agents and FMOs

MMI+ members can also download a simple LTV calculator to model the different commission structures.

[Note: this analysis is only looking at maximum agent/broker commission. It does not consider override commission.]

What Changed

Historically, Medicare Advantage commissions have followed a simple structure.

Replacement / Switch

Prorated based on enrollment month

Humana recently updated its structure (effective April 1, 2026) to prorate commissions on NTM policies.

This means:

Effective Month Initial Year Commission Proration

January

April

October

November

The change effectively shifts a portion of the first-year commission out of the system.
