Medigap Market Updates: June 2026

This week’s deep dive is a continuation of the Medigap Market Update series and will cover recent happenings in the Medicare Supplement Insurance market.

Product Activity

No new carriers entered the Medicare Supplement market in Q2 2026.

Market Experience

Back in April, we provided early Earned Premium and Incurred Claims estimates for the Medicare Supplement market in 2025. We now have the most recent NAIC data based on the reported Medicare Supplement Experience Exhibits.

  • 2025 total market Earned Premium has been revised from $39.9B to $41.9B.

  • 2025 total market Incurred Claims have been revised from $34.1B to $35.9B.

  • The 2025 loss ratio increased from 84.5% in 2024 to 85.5% in 2025.

The following chart shows the 2024 and 2025 reported loss ratios for the six largest carrier groups in the Medigap market today: Aetna/CVS, the Blues (“BCBS”), HCSC/Cigna (“HCSC”), Humana, Mutual of Omaha (“MoO”), and United Healthcare (“UHC”). All other carriers were combined and are shown as “Other”. The reported loss ratio for the entire market in 2025 was 85.5%.

Rate Increases

Early analysis of 2026 open block rate increase filings show approved increases continued to rise compared to prior years for most plans.

  • The exception is for Plan F, where 2026 rate increases have been slightly lower than in 2025.

The following chart shows the average rate increases on open Medigap blocks for Plans F, G, N and HDG over the past three years.

Updated Medigap Claim Trend Estimates

2025 loss ratio experience with claims runout through Q1 2026 suggests that Medigap claim cost trends may have continued higher in 2025.

 Plan F and Plan G medical trends have been ranging from 9-14% over the past 3 years.

 2025 experience is showing medical trends around 12%.

 Loss Ratios continue to be elevated, as carriers wait for recent larger rate increases to have a more meaningful impact.

Regulatory Updates

Maryland has expanded open enrollment opportunities for individuals losing state medical assistance coverage or qualifying for federal special enrollment periods. This new rule is effective on 7/1/2026.

 New Mexico has introduced a new Birthday Rule enrollment window. The effective date for this rule is 1/1/2027.

West Virginia has also introduced a Birthday Rule enrollment regulation that became effective on 6/11/2026. The state has also included a regulation for a guaranteed issue period following the loss of Medicaid eligibility.
